TERMS AND CONDITIONS
ASSUMPTION OF RISK
I understand that participation in martial arts activities and competitions—including but not limited to sparring, grappling, forms, flag sparring, and drills—involves inherent risks of injury. These risks may include, but are not limited to, bruises, strains, sprains, fractures, concussions, and other serious injuries.
I knowingly and voluntarily assume all risks associated with participation in this event.
RELEASE OF LIABILITY
In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless Pinnacle Martial Arts, its owners, instructors, staff, volunteers, referees, event officials, sponsors, and facility owners from any and all claims, demands, actions, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur during or as a result of participation in this event, whether caused by negligence or otherwise.
MEDICAL RESPONSIBILITY
I certify that I (or my child) am physically capable of participating in this event and have no medical conditions that would prohibit safe participation. I understand that Pinnacle Martial Arts does not provide medical insurance for participants.
In the event of injury, I authorize Pinnacle Martial Arts staff to obtain emergency medical treatment if deemed necessary, and I accept full financial responsibility for any medical care received.
RULES & CONDUCT
I agree to abide by all tournament rules, safety regulations, and referee decisions. I understand that unsportsmanlike conduct, unsafe behavior, or failure to follow instructions may result in disqualification without refund.
PHOTO & VIDEO RELEASE
I grant permission for Pinnacle Martial Arts to use photographs or video recordings taken during the event for promotional, educational, or marketing purposes without compensation.
MINORS
If the participant is under 18 years of age, this release must be signed by a parent or legal guardian, who agrees to all terms on behalf of the minor.
GOVERNING LAW
This agreement shall be governed by and interpreted under the laws of the State of Washington.
